Eastern Grove Support LLC will review the referral and assess the individual’s needs to determine the most appropriate level of care. If another service is clinically indicated, our team will assist with coordinating an appropriate referral whenever possible.

Supporting documentation may include recent assessments (H&P, Comprehensive), medical records, medication lists, discharge summaries, laboratory results, treatment recommendations, and other relevant clinical information to assist with the admission process.
